Tattoo Request Form

~Please expect a 3-7 day response time . If you haven’t received a response after 10 days please re-send your submission or check your spam folder ~

Please fill out all required boxes with as much detail as you can.

Once I respond to your submission you can reply and upload any reference and inspiration photos you may have.

After discussing an ideal time for the appointment I will require a $50 ( or $100 for large scale projects) non-refundable deposit through PayPal or cash in person.

The deposit will secure the date and go towards the total price of the tattoo.

If I don’t receive the deposit within 48 hours after requested, I will assume you aren’t booking and will forfeit the date.

If you need to reschedule you must give me a 3 day notice, otherwise your deposit expires and I will require another one to re-book.

Custom designs are sent out the night before the appointment.

The day of the appointment please arrive well rested and eat beforehand as you will be sitting for a few hours.

If you are going to be more than 20 minutes late to your appointment I will consider it a no-show.

I will require a valid unexpired I.D. for paperwork. I can accept military I.D., state I.D., passport, or drivers license. I can accept birth certificates only with a photo I.D.

Cash is preferred for payment but I can accept PayPal or card.
